Vaccination Clinic Survey

The Diocese of Springfield Pandemic Response Team would like to determine if any of our parishes are interested in offering their premises as COVID-19 vaccination sites, in the event there is a need. All parishes will be provided with liability coverage and a Facilities Use Indemnity Agreeement. Kindly let us know if you are interested by filling out the form below.
