Certificates of Insurance

To request a Certificate of Insurance for General Liability or Property, you must fill out the GL/Property Certificate Request Form email it to Michael Polizzi at michael@wrs1928.com. It is helpful if you can also send the expired/expiring Certificate of Insurance for reference.
